Before you start

  • Answer as fully as you can. If you are not sure about something, tick it and we will go through it with you.
  • Your answers are sent securely to the clinic and are only used to check that treatment is safe and suitable for you.
  • You will still be asked to confirm your details and sign a consent form in person before any treatment.
  • If anything changes before your appointment, such as new medication or a tan, please let us know on 01554 227543.
